It depends entirely on where you live, and the differences are larger than most national guides admit. We read the code, ordinance, or fee schedule published by each of 137 US counties and cities and recorded what it actually says.

Of the 137 jurisdictions where we have confirmed the rule, 102 require a permit outright, 35 require one only in certain conditions.
The most common published threshold is 5 tons, used by 2 of the 3 jurisdictions that state a threshold in tons. That leaves 1 of them on a different number, which is why a national rule of thumb is not a safe answer for your own address.

What an HVAC permit costs
131 of our HVAC permit rows carry a fee answer taken from the jurisdiction's own published schedule, across 131 localities. There is no single number to quote, and not because we did not look: jurisdictions price this permit in genuinely different ways. Some charge a flat fee, some a percentage of the job's assessed valuation, some a rate per square foot, and some do not price it at all because the work is permitted by a state agency or a utility instead. Each row below is that jurisdiction's own wording.

How HVAC permits work, start to finish
Applying
A residential HVAC (mechanical) permit application must identify the covered equipment and scope of work, since installing, altering, or replacing a heating or cooling system is regulated work that cannot start until the application is submitted and the permit issued. The equipment itself must be picked by a real sizing calculation rather than a rule of thumb.
- A mechanical permit is required before anyone may install, alter, repair, remove, convert, or replace a heating, ventilating, or air conditioning system regulated by the code, and the work may not begin until the application is submitted and the permit is issued (IRC R105.1).
- The application identifies the specific equipment being installed, such as a furnace, heat pump, or air conditioner, since the unit must be installed according to its listing and the manufacturer's installation instructions (IRC M1401.1).
- The proposed equipment location must leave the clearances needed to clean coils and replace filters, blowers, motors, and controls after installation (IRC M1401.2).
- The application carries the equipment sizing basis: ACCA Manual J building loads and Manual S equipment selection, or another approved sizing method, not a rule of thumb (IRC M1401.3).
- For a split system, the work covered by the permit includes refrigerant suction line insulation, since the piping is inspected against that standard once installed (IRC M1411.6).
Inspections and close-out
Mechanical work gets a rough in inspection after the ductwork, refrigerant lines, and gas piping are in but before they are covered or concealed, followed by a final inspection that confirms the equipment operates correctly and its clearances, combustion air, and condensate disposal are in place. A straight equipment changeout with no new ductwork commonly gets a single final inspection instead of a separate rough in trip.
- Rough inspection of the mechanical and gas systems must be made before the work is covered or concealed, before fixtures or appliances are set or installed, and before the framing inspection (IRC R109.1.2).
- Where a cooling coil is added to an existing warm air furnace, the rough in confirms the coil is not installed upstream of the heat exchanger unless the furnace is listed for it, and that the blower can handle the added static pressure (IRC M1411.2).
- The rough in and final both confirm the appliance access opening and working space are large enough to service the unit (IRC M1305.1).
- At final inspection, the inspector verifies the equipment has adequate combustion air, with openings kept clear for the distance the code requires (IRC M1402.3).
- Final inspection also checks that the condensate line drains to an approved point of disposal with the required slope and does not create a nuisance (IRC M1411.3).
- For a straight equipment changeout with no new ductwork, many permit offices require only a single final inspection, while a new installation still gets a rough in inspection before the final (common local inspection practice).
